I don't know about the worst, but one of the most difficult to say must have been the one in Return of the Archons, by Spock: "We are not Archons, Marplon."

Makes my jaw ache.

In TMP, (a movie I love BTW) we get this exchange when Spock returns to the ship and is lying in sickbay:

Shatner: Spock

Kelley: Jim

Shatner: Bones!

That's dialogue?
An economic translation of;

Kirk: "Spock, I need to know what happened out there."

Bones: "Jim, he's in no fit state to answer questions."

Kirk: "Bones, this is an emergency! I don't have time to coddle him..."

Also more combination of a situation and dialogue. Whenever an alien manages to bypass security controls get a shuttle and mask their warp trail. The responses to the captains various orders are 'i can't', 'i've been locked out', 'they're gone'. No one ever seems astonished that trained crewmembers who were able to secure a post on an advanced starship would fuck up so royally.

It happend so frequently on Voyager and is just such horrible, lazy writing. Janeway was never like 'what the fuck Harry, you're incompetent in every fucking episode'. 'Here's a formal warning to get your shit together or you get demoted to janitor'.
There's no point in blaming Kim when it was already a cliché that had been well-established by TNG. Whenever somebody stole a shuttle, Data would report it, push a few buttons to try to stop it, and fail. Makes you wonder why they even had the option to stop it if it never, ever worked, even when performed by what had to be the fastest Ops officer in the fleet.

I didn't mean to blame Kim, i was just using him as an example as i'm more familiar with voyager then tng. Regardless of who is doing it, it's still annoying.

Data was trying to be human after all.

Whoever designed the fail safes on the anti-matter containment should be shot.

Not to mention the shoddy design of the always-failing starboard power coupling.

Anytime L'waxana Troi gets sexual, at all is dialogue that should've ended up on the Cutting Room Floor.

For me is got be this exchange from every star trek incarnation ever:

random crewmember: 'bridge to captain'

Captain: 'go ahead'

random crewmember: 'there's something here you should see'

Whilst i get it's a conceit so the audience doesn't know what to expect but it just annoys me. The Captain never does what any normal person would do and say 'what is it'.
Good call! Seven seemed to use this one constantly, I think Janeway gave at least one of those "Seriously" looks in response.

And auxiliary power. Why the hell did the backup systems always seem to fail at the same time that the primary systems went offline? Isn't the purpose of a secondary system to ensure that a ship keeps functioning even if several primary systems have failed?

Warp plasma coolant is another problem I've noticed a lot. Why is always leaking? Why can't Starfleet engineers design a better storage space for it?

L'waxana Troi in general should've ended up on the Cutting Room floor.

The "starboard power coupling" did get noticeable as time went on... and noticeable that the characters never seemed to notice how often that same damned power coupling was screwing them. Like, why not try taking some design tips from the port power coupling? That one seems to hold up relatively well...

Spock's "Khan!"

The only time I;ve ever been embarrassed about being a Trekker.
I thought so too at first watch. The second time I watched, I noticed ha Quinto's yell sets up the next shot, in which Khan and Vengeance plummet past Enterprise on their way to the San Francisco crash sequence. It's actually very well done... we'd almost forgotten they were still out there.
