I always figured Janeway just did not have enough pips to go around, and for whatever reason the replicator could not replicate them (maybe it was a General order from Starfleet, a way to cut down on counterfeit pips).
What about the officers Janeway promoted, like Tuvok?

They had a few dead bodies when they arrived in the Delta Quadrant. Spares for promotions, but not enough to fit Chakotay's crew in full.

Janeway probably had enough to do maybe 10 promotions through a year, and then get more upon return to space dock. In one year Janeway wasn't going to promote every single crew member on the ship, so she wouldn't need a pip for everyone; her quantity was probably only big for 1/11th of the ship. Look at Harry Kim---he was an Ensign for all 7 years; if I remember Datalore correctly, Starfleet officers were only Ensigns for about 2 years before being promoted to Lieutenant (Jr.) grade.

I haven't thought so much about Tom not having a Maquis rank pin, I've wonder why the Maquis didn't get Starfleet rank pins.

Some of them had been in Starfleet. Chakotay was a Starfleet Lieutenant when he left Starfleet and joined the Maquis and since Tom got his Starfleet Lieutenant rank, it would also have been logical to promote Chakotay to a Starfleet Commander with a Starfleet rank pin.

beides that, Janeway was very clear on the crew being a Starfleet crew so why theh had special Maquis rank pins is a bit odd.

Not only that, Kes and Neelix should have been accepted as Starfleet personel too. Kes did actually learn everything to be a Starfleet nurse and Neelix, well wasn't he the Morale Officer.

As for Seven, she did get a Starfleet uniform after some years on the ship.

Maybe the Maquis crew members chose to have a slightly different rank insignia, and Janeway went along with it to keep the peace?

Seven's Starfleet uniform was only in the Holodeck (and on her time travel mission, in order to blend into the crew).

But with Neelix and Kes I don't think they needed uniforms, since we saw Guinan (who was the Head Bartender/Morale Officer on the E-D), Mr. Mot (ship's barber) and Keiko (as a ship botanist) in civilian clothes in TNG.

I think I'd have liked Kes to switch to a uniform if she'd not left. I wish Seven had switched after we saw her desire to wear one. She clearly wanted one, but she seemed to have some sort of need to distance herself from the crew, and admitting to that outside of the holodeck was just beyond her.

But that would've gone against Starfleet's "young lady in a cat suit" mandate.

Oh, right. General Order 3, isn't it?

I thought it was General Order 1, Subsection H, being an inclusion after the stint of T'pol of Vulcan on Enterprise.

What, no one's gonna chime in, calling it General Order 69? Geez!

That be way too late in Starfleet history to justify.

I always enjoyed those episodes with Neelix in a Starfleet uniform, its a shame it only ever happened in alternated timelines.

Kes would have suited a Starfleet uniform as did Seven, really they should have gotten her into one of those regularly by the end of the show but as with Enterprise TPTB wouldn't have been happy with their shows 'babe' not wearing a figure hugging outfit.

Re: Why didn't Tom Paris wear a Maquis rank pin?

Kes, Neelix, and Seven were all civilians, so officially aren't entitled to wear a Starfleet uniform.

Regarding the speculation on regulations regarding catsuits, that's actually General Order 36-24-36.
