I'd add in several things from The Wrath of Khan
: Terrell's suicide, the Ceti eel coming out of Chekov's ear and one of the Enterprise crew being turned into a human torch when Reliant hits it in the torpedo bay.
In my opinion, The Wrath of Khan
is the most graphically violent film in the series.
