In this case, Carter was not the target of the assassination (Reese was, she just got in the way), and there was no advanced discussion of her murder, so the Machine didn't see it coming at all or didn't see it coming until it was way too late to do anything about it.
I think it's assumed that Carter's number did come up. That's what the phone was ringing for at the end.
