That was a costly operator's error. It would otherwise not have happened at all if someone had been paying attention that the
Enterprise's transporter wasn't working right--it's the reason why Kirk boarded the ship by travel pod. Any other time, if the transporter was faulty, no one would be beamed anywhere until it was fixed as was the case both before and after that incident.
